Autumn Leaves

I recently visited my friend at Autumn Leaves and wanted to share a positive review online. He could not be happier! He says the people there are really nice and the food is almost always very good. Thank you for taking good care of him.

Staff members are always very professional and willing to go out of their way to help residents and families. . Kind, friendly and caring atmosphere. Excellent location for anyone looking for an East Dallas location.

They provide great care! Great staff and administration. Clean facility. I would suggest anyone to tour this facility! Great location and proximity to downtown dallas. Beautiful views of White Rock lake!

I have had such a wonderful experience with the staff here. They have been attentive and very responsive. The DON and Social worker have been amazing to work with and go above and beyond for their patients and residents. I highly recommend!

It was bought by somebody new, but it appears the folks are doing their best but sometimes there seems to be a lack of communication with the families. That can be frustrating when you do not live in the same city as Autumn Leaves. They do a good job at calling me, but if I call back it is hard to get someone on the phone. Overall I have been satisfied and they have been accommodating. It is a very good value for the money especially in the area.

The hallways and elevators are not cleaned properly. There is a foul order in the hallways.

My 91 year old dad spent almost a month at Autumn Leaves for mobility support after a fall at home. He received great therapy while there and the team of care providers were professional and compassionate. [name removed], in particular, went above and beyond and gave us so much support. We are grateful for the care he received. My only suggestion is that the facility could be better maintained. But the staff and experience was positive and we appreciate that.

My mother was in skilled nursing from Oct.27-Nov.23 2021. I had done a good bit of research and Autumn Leaves had gotten excellent scores from Medicare and other non biased sources. They did not disappoint. The facility may not be the shiniest but the cleanliness and professional staff more than made up for it. My family and I came often and saw firsthand the patience and kindness of the staff. The nurses were quick to address Mothers pain and treated her with dignity & respect. Mom had injuries from a fall so physical therapy was essential. The PT staff were excellent & a big part of Moms recovery. The Director of Nursing was available to us to answer any concerns at any hour and that really made our family feel comfortable. It is hard to relinquish care of a loved one to others but I can sincerely say our time at Autumn Leaves was the best possible experience. We are a grateful group!

I have had the privilege of working at Autumn Leaves for almost 2 years. We have an incredible therapy team who really focus on allowing each patient to reach their full potential.

The staff works very hard to make the residents comfortable except at nights. Don’t know if they are over worked or just don’t care, my Mother fell after trying several times to call an attendent and tried to go to the restroom by herself. She fell and broke her leg which has not healed, but they haven’t checked it in over a year for healing. The staff is short handed but do the best they can, but they need more help. The food could use a little more variety, though I know you are limited by many dietary limitations. On the whole I think you do okay, though admin needs compassion training!

the independent living residents seem to really enjoy being a part of the Autumn leaves community.. the calendar is full of activities including a weekly shopping trip to Walmart... staff is helpful and courteous..

The staff at Autumn Leaves are wonderful, caring people who put residents needs above their own.

Our first impression of Autumn Leaves was great, the receptionist is outstanding and very welcoming. On our tour we were able to see different apartments which are spacious and the closet space is excellent. It is an older facility but it has its pluses and it is reasonably affordable. They have had some pretty good activities lately like outside classes and entertainment. The community overall is clean, but they could improved on the frequency of how many times the house keepers come in a month.

We're still getting acclimated. The food is getting better, which is nice. But I still bring my bag of spices down to meals with me. We are slowly getting things done as a community. That was stalled for a little while when our director had an accident and had to have surgery. But since then, it has improved and things are moving again. We now have a fire escape plan and a tornado plan. My husband and I have met some lovely people here. And the staff are awesome. From my balcony I have a view of White Rock Lake, which is very much a part of my life and the gem of Dallas, so that's been nice. Overall, we are enjoying living here.

Very impressed. Older remodeled facility with large apartments and great views. Looks like a place I could enjoy living.

The heater didn't work, the oven didn't work in my loved one's room. It took three weeks to get the freezer door fixed. They promised us a washer dryer hookup and they didn't have them in my loved one's apartment. There was no housekeeping as promised. They would forget to deliver food to my loved one. I like the community and it was beautiful initially. I would meet the management team before putting a loved one in the community.
