A great place for mom

We really like the size of Park Senior Villas, each of the homes have about 10 residents or so in them so the staff members are really able to get that personalized one on one attention to our loved one and the other residents. They have been great working with our loved one - she loves to dust and set tables and whenever our loved one was getting agitated they could redirect her attention and let her set up the dining tables or sweep the floors. It really makes her feel included which we appreciate. They have a lot of activities on their schedule but they don't always do what is on the schedule. Our families experience was a disaster, our loved one had bruises on their body, they were never fed, and was severely dehydrated. Our loved one never received medication and when they did it was the wrong medication.

What a great concept. Park senior villas goodyear has a great concept. Follow through with care needs is another matter. After multiple conversations with staff and after witnessing State ombudsman out on several occasions during may and june family members learned manager wasnt licensed and facility was in danger of license loss after not having license for nearly a year. State of AZ has some atoning to do. Some of the direct care workers are the sweetest, attentive people but are limited in scope. Medication management- is questionable- acting /manager and her family members (nurse and office staff) are in the money game. Breech of trust are regular occurrence, cleanliness and hands on assist is minimal. Chemical restraint is probable to make up for lack of manning. Isolation is probable for same reason. Facility manager will push for outside agencies / medicare funds to offset resident needs through “preferred partners”. Redflag.

A great place for mom

A Wonderful Place!

I currently have my Mom at the Park Senior Villas in Chandler. They do an amazing job. I know everyone has different expectations, but I am extremely satisfied. Everything is kept sparkling clean. The activities director does a wonderful job. It's hard when so many people are compromised, but she goes out of her way to involve people. Yes, occasionally clothes go missing, but if brought to their attention, they are located and brought back. The people there really care, and God knows it is a thankless job. I give them high marks and wouldn't hesitate to recommend them to anyone. From [name removed] the sales manager to [name removed], the Director in charge, to [name removed], the medical director, they all rock! The attendants are all super respectful of the residents and families. The food there is delicious. I checked out many facilities before deciding on this one. None will ever meet every expectation, but you can find good ones, and this one is.

A Good Place for Someone That Needs Extra Care

They seem to be taking good care of my loved one. The community is able to assist with caring for my loved one with medication management, bathing, dressing assistance and more. The monthly rate includes the care, the room, utilities, laundry, changing of linens, transportation and all the meals are included. My loved one seems to enjoy the meals. The community does provide a list of activities available.

My tour review

When it came down to it we just went with another community that we felt met my loved ones needs better. The tour was excellent, there were awesome people running this place. I was looking for the level of the resident and what their level of care was in comparison to my loved one to make sure that she had someone around her also that was like her.

small family atmosphere

unless location is an issue, i would advise others to look elsewhere. during the first three months, they had an activities director who made sure my loved one was involved in activities, as well as the other residents. she left around Christmas time and it seems that they have not made an effort to find a replacement or create activities themselves. im unsure if her absence or the lack of activities are due to Covid. regarding the care services, i'm unsure if the room is being cleaned on a once a week basis as it should be. they should also make it mandatory to assist the residents with showering more than once a week to maintain hygiene - especially those with a more advanced level of care. i did like some of the staff because they were always very friendly and attentive to my loved ones needs. part of the issue with management is that they were very laxed with Covid mandates and protocols. they did not enforce wearing masks amongst the staff and visitors; and i did find that a little careless.

Don't move your family member here

There were one or two good employees. Otherwise, I witnessed some disturbing things when I would visit from out of state. For instance, I was watching them dish up food one day. And the gentleman who was dishing it up put the food on the residents' plates and then reached into the serving trays with his bare hands, eating the food out of the trays. Meanwhile, the residents were welcome to come back for seconds out of those trays. My sister, who lived there locally, had issues talking to the staff about anything new that came up regarding medical care. She would have to tell them over and over about any changes. They went through management so quickly there, you couldn't even keep up. The food was absolutely terrible. Some of it wasn't even recognizable. They were supposed to offer another option if you didn't like something that was being served, but any time my loved one would ask for something else, they would just say "This is your dinner." They didn't have activities very often, despite telling us they had them every day. The last straw was when my loved one called my sister and said "There's nobody here to help me. What do I do?" One of the other residents' family members received a similar call and then called 911. When 911 showed up, there was literally not a single employee on site in the memory care house. They had just completely abandoned the residents. We had to wait months to move our loved one out due to Covid, but we finally did get her out.

Please Read

On [date removed] , my husband was admitted to Park Senior Villas, Goodyear, AZ. after much research of Memory Care Facilities. I had several conversations with the Marketing Director at Park Senior Villas as well as a lengthy meeting with their Licensed Practical Nurse, where we discussed at length my husband's current medications, the daily care he needed, his current medical conditions and medical history. The nurse took notes and did cognition tests on my husband. In addition, medical records were sent to the Marketing Director from three of my husband's physicians currently caring for him which were his Internist, Neurologist and Urologist. I was also told by the Marketing Director that their caretakers were required to take an Alzheimer's course to familiarize them with recognizing the needs of their residents. I completed the required paperwork from the facility, including a sheet of my husband's daily needs which stated: Foley catheter care, blood pressure checks, notification of meals, medication administration, help with toileting, bathing, dressing, ambulation (I sent a walker and shower chair with him,) and I noted he was shuffling his feet and had fallen several times, confusion, some hearing loss, speech, special diet (no tough meat), oral hygiene, face shaving, etc. I also met with Sage Hospice, Scottsdale, AZ, where this information was reviewed with them, and I arranged for their care as well. I was told by hospice, that initially a registered nurse would see my husband twice weekly under their physicians' review and approval. I was also assured by the Marketing Director at Park Senior Villas as well as the hospice nurse that if anything at all changed with my husband that I would be contacted immediately. Feeling comfortable with the facility and the level of care that was going to be provided to my husband, he was admitted on [date removed]. He was in isolation (COVID 19 mandate) for 2 days and I was told he'd have frequent caregiver checks. The Marketing Director advised that visitation was limited due to COVID 19 and that they strongly advise that loved ones not visit for two weeks to allow the resident time to acclimate to the facility which made it much easier for the resident. I was told that I could call anytime on the caregiver line to check on my husband, which I did, calling different shifts. I was told he was "good" "going to his room with an ice cream sandwich, "eating well", doing o.k." I scheduled a visit to see my husband on [date removed]. On [date removed], I received a call from the Hospice Nurse who routinely called weekly. She said my husband was "doing o.k." but that he had a "cough", didn't want to stay still and was "sleepy" so she didn't check his blood pressure, she also said that she was told "he hadn't had much of an appetite the last several days." She again said he was o.k. and that the staff was going to "get him into the shower" later that day". She said she'd see him later in the week. A little later, I received a call from the facility asking if I had a "Do Not Resusitate" on my husband. I asked what was wrong and was told his blood pressure was very low 60/40. I told the caller to immediately call 911 to have my husband taken to the nearest hospital. My daughter and I immediately got in the car, I called the facility to find out the name and address of the hospital and we arrived just as they were wheeling my husband into the emergency room. When I was allowed to see my husband and speak with the doctor, he said my husband's blood pressure was extremely low, said he had pneumonia and was severely dehydrated and weak. He said they were treating him with IV fluids and antibiotics, that he ordered a battery of tests, was going to see the kidney specialist who was in to see if he was a candidate for dialysis, and that he would check with me after all the test results came back. Later that evening, I spoke with the night doctor on call. He said my husband was not a candidate for dialysis, said his kidney function tests were so high "they were off the charts." He said they were doing everything they could to care for him but that he was very, very ill. The doctors treated my husband for four days, did additional testing and scans but came to the conclusion that as aggressively as they were treating him, and that while his tests had improved some, he was not going to get better. On [date removed], my husband was discharged from the hospital and brought home via ambulance and cared for by another hospice group. He passed away 28 hours later on [date removed]. When I saw my husband after he was admitted to the hospital on [date removed], he had lost a lot of weight, looked extremely pale, was wearing a dirty shirt; the same shirt he was wearing when he was admitted on [date removed], and the same shirt he was wearing in a picture sent to me by the Marketing Director 4 days after his admission, and he had not been shaven in days! Looking at a picture of him arriving at the facility on [date removed], versus the picture of him when he was admitted to the hospital on [date removed], he was practically unrecognizable. I am grieving over the loss of my husband of 52 years, and also over the lack of care and neglect he received from Park Senior Villas, and Sage Hospice.

Don't be fooled by one visit.

At first glance this seems to be a great place for an elderly father that just needs some help is his day to day life. I did a day and evening check in twice. On both occasions the morning crew was working and doing what should be done. During the evening I saw not one caregiver the first time. The second time it seemed as if I woke up the caregiver that was supposed to be taking care of the people. After being almost completely convinced to not place my father there I decided to go back one more time. I asked a caregiver why there were hardly any caregivers at all. I was told that the caregivers never stay long because the patients say mean and hateful things to any one of color. And that no one ever helps them. If the staff are treated crappy you can bet they wont stay or they wont treat the people living there good. I was glad I didnt put my dad there.

Beautiful for her last days

When we first took my mom there it was a different company, my issue was when the ownership changed, They never told us, we just noticed it. and how we noticed was that there was a huge turnover in staff. When I asked the one staff person left that we knew, she said that the company changed hands and she was frazzled over it. My mom has since passed away, The facility is beautiful, the food was very good, and it was very clean.

RUN AWAY TERRIBLE PLACE

On the surface Park Senior Villas looks great but in reality it is a mess. In the 4 months my mom was there she received 3 showers. They med techs failed to administer her medication on so may occasions I lost count. The residence were left unattended in the evenings with no care givers in the home. On two occasions there was an entire day where there was not even one care giver in the home. We had multiple meetings and were told things were going to get better, we even had a meeting with the owner and no changes were made. Things actually got worse. The police and fire department had to be called on multiple occasions due to no staff being on site and family members not being able to get into the home after a resident fell and was laying on the floor for over 2 hours when there was no care takers in the home. With the cost being $5000.00 a month just for room and board all we wanted was basic human care. Run away from this place. I would not recommend placing any family members here. The list goes on and on with the lack of care we received at Park Senior Villas.

I toured this community for the care for my mom. The staff were nice and able to answer the questions we had on te care that is offered here. The community was clean and they offered a good variety of healthy meals. We liked that the menu was appealing. We would suggest that other families tour this community for the care of a loved one.

Loved the way this location showed. Grounds were not as beautiful as some other locations but the feel was much more homey. The staff was nice and the residents seemed happy and well adjusted. The price was average and the option for medicaid after a few months was a plus.

I'm not so good at rating.

I'm not so good at rating things but what I can say is that the experience I've had with Pathways has been great! My Dad is still trying to adjust but in the process he has participated in some activities and maybe made a friend or two. He really enjoys the food and the cooking is the only odor you'll smell when you go to Pathways. I'd absolutely recommend them to a friend!

Great Alternative

We are very happy with my mother's move into Pathways things are going great for her here. My mother has been a clean and neat freak all her life and one thing she always says to me is how clean the community is and how nice it looks . The staff here has been wonderful and nothing but very helpful and kind to us. One thing my mother seems to enjoy a lot is the food here she loves it and has never said one bad thing about it. She just loves it here so far they have good activities for her to do if she feels like it and she has made some new friends as well so that is really nice to. I have all ready recommended this community to a neighbor who's husband has been showing some early signs of dementia I told her this is a very good alternative for us and helped us out very much.
